Monday-Friday 8am to 5pm
What You’ll Do
- Coordinate discharge and aftercare plans with hospitals, crisis units, and community partners.
- Perform intake assessments and connect individuals to essential mental health services.
- Develop person-centered plans and provide crisis interventions and safety planning.
What We’re Looking For
- Experience: 3 years in community mental health or related fields.
- Skills: Strong interpersonal, documentation, and time-management abilities.
- Flexibility: Able to work non-traditional hours and handle high-pressure situations.
