Central Michigan 2-1-1 is seeking a Community Navigation Specialist to provide one-on-one support and guidance to individuals navigating health and social service systems. Rooted in the Community Hub/Pathways model, this role is all about empowering people to improve their lives—one connection at a time.
What You’ll Be Doing:

Provide confidential and unbiased navigation services for individuals and families seeking help with health and social services.

Help clients identify and overcome barriers impacting their Social Determinants of Health (SDOH).

Screen individuals for social needs, capture insurance and billing details, and offer appropriate referrals and advocacy when systems feel overwhelming.

Coordinate and attend face-to-face client appointments with agencies and health services—primarily in Jackson County, but with occasional travel across Michigan.

Maintain accurate, up-to-date data on services, eligibility requirements, and client outcomes.

Build strong partnerships with community organizations, health care providers, and LifeWays staff to ensure coordinated care.

Offer follow-up support to ensure each client’s needs are being met and no one falls through the cracks.

Stay up to date on state and local policy changes, especially around Medicaid and social support services in Michigan.

Familiarity with AIRS standards, CARF accreditation, or MDHHS programs is a plus—but not required.

A valid driver's license and ability to travel locally as needed.

Required Education/Certification/Licensure
- Associate’s Degree in Human Services or related field, or 2 years relevant experience.
Preferred Education/Certification/Licensure
- Bachelor’s Degree in Human Services or related field.
- AIRS Certified for Community Resource Specialist.
- Bilingual
