2-1-1 Community Navigation Specialist

Central Michigan 2-1-1 is seeking a Community Navigation Specialist to provide one-on-one support and guidance to individuals navigating health and social service systems. Rooted in the Community Hub/Pathways model, this role is all about empowering people to improve their lives—one connection at a time.

What You’ll Be Doing:

• Provide confidential and unbiased navigation services for individuals and families seeking help with health and social services.

• Help clients identify and overcome barriers impacting their Social Determinants of Health (SDOH).

• Screen individuals for social needs, capture insurance and billing details, and offer appropriate referrals and advocacy when systems feel overwhelming.

• Coordinate and attend face-to-face client appointments with agencies and health services—primarily in Jackson County, but with occasional travel across Michigan.

• Maintain accurate, up-to-date data on services, eligibility requirements, and client outcomes.

• Build strong partnerships with community organizations, health care providers, and LifeWays staff to ensure coordinated care.

• Offer follow-up support to ensure each client’s needs are being met and no one falls through the cracks.

• Stay up to date on state and local policy changes, especially around Medicaid and social support services in Michigan.

• Excellent communication skills—on the phone, in person, over email—with people from all walks of life.

• Comfort and sensitivity in working with individuals from diverse backgrounds and medical or social conditions.

• Familiarity with AIRS standards, CARF accreditation, or MDHHS programs is a plus—but not required.

• A valid driver's license and ability to travel locally as needed.

Required Education/Certification/Licensure

• Associate’s Degree in Human Services or related field, or 2 years relevant experience.

Preferred Education/Certification/Licensure

• Bachelor’s Degree in Human Services or related field.
• AIRS Certified for Community Resource Specialist.
• Bilingual
