POSITION TITLE: Program Assistant/Intake Specialist
CLASSIFICATION: Full time; regular; non-exempt
SUPERVISOR: Program Manager
POSITION SUMMARY:
The Intake Specialist is primarily responsible for providing a process of triage and access that ensures proper placement of all participant families attempting to access services. The Intake Specialist is responsible for conducting the initial assessment, assisting the participants with completing the intake assessment, verification of homeless status, and relevant data collection. The Intake Specialist must enjoy working as a member of a team in a cooperative effort and be able to exercise creative and independent judgment to solve problems. The Intake Specialist is expected to perform her/his duties in alignment with the vision and values of LightHouse Social Service Centers.
DUTIES AND RESPONSIBILITIES:
1. Processes telephone and walk-in inquiries by assessing needs, scheduling intakes, and collecting referral source data.
2. Evaluates prospective participant families in the intake interview for appropriate referral to program services.
3. If the program services are not appropriate, will provide participants with a minimum of three community referrals.
4. Will process all program admissions per program policy.
5. Conducts follow-up calls to viable participants as well as to all “no show” participants.
6. Maintains relationship with community agencies that refer clients to the program and contributes to the on-going development of new referral sources.
7. Maintains the program waiting list.
8. Completes all intake paperwork with participants.
9. Assigns participants to a case manager.
10. Enters HMIS data and ensures that data is correct and up-to-date.
11. Assists with receptionist duties as needed.
12. Complies with all policies and procedures.
13. Performs other related duties as requested.
POSITION REQUIREMENTS:
a. AA degree in Human Services preferred, or one (1) year experience working in the field of homeless services or related human services experience.
b. Knowledge and experience with homeless and communities.
c. Knowledge of community resources and services
d. Strong telephone skills.
e. Strong decision making skills and good judgment.
f. Strong computer skills and organizational skills.
g. If recovering, a minimum of two years sobriety.
h. Valid driver’s license and insurable.
J. Experience with culturally diverse populations.
Job Type: Full-time
Pay: $24.00 - $26.00 per hour
Expected hours: 40 per week
