We are currently seeking Per Diem or Fee-for-Service Assessment RNs

RN will conduct comprehensive initial and recertification assessments for a Licensed Home Care Service Agency (LHCSA) and develop a plan of care in accordance with the physician’s orders and with the policies and procedures of the agency, the state’s professional standards, and regulatory entities.
Responsibilities:
- Assess/Re-assess patients in their homes.
- Perform change in condition visits
- Supervise/assess Home Health Aides in the patient's home.
- Educate patients about health maintenance and disease prevention.
- No UAS
Skills / Requirements:
- NYS licensed and currently registered as a Registered Professional Nurse
- The ability to work independently and within a team.
- Ability to multi-task and prioritize tasks and must be flexible.
- Optimistic, energetic, outgoing with great energy.
- Strong organizational skills
- Strong interpersonal communication skills - with internal and external clients
- Microsoft Office(Preferred)
- Homecare experience: (Preferred)
- Able to multitask and work in an extremely fast-paced environment
- Excellent customer service having empathy and patience in working for the elderly population
- Spanish speaking (not required)
