What are the responsibilities and job description for the Service Coordinator position at LinkAbility, Inc.?
Essential Responsibilities & Duties:
1) Conduct personal plan meetings, face-to-face, with eligible individuals with developmental disabilities.
2) Work with the individualized support team to request needed services that cannot be met using natural support.
3) Maintain contact with every individual on caseload as per Department of Mental Health (DMH) Directive 3.020, Individual Support Plan Monitoring and Review.
4) Maintain a varying caseload of 35 or more consumers based on the agency’s needs.
5) Complete logging documentation to support billing for Targeted Case Management on a daily basis. Service Coordinators are expected to log 120 billable hours per month minimum. a) Annual minimum requirement of 1,440 billable hours per year per TCM Contract.
6) Meets or exceeds all deadlines in an accurate manner.
7) Complete and maintain consumer’s State records in accordance with DMH, Medicaid, and agency guidelines.
8) Attend all mandatory staff meetings and relevant training(s) as required. Implement new information.
9) Provide after-hours on-call services to consumers, their families and service providers, rotating responsibility weekly with other members of the Service Coordination team.
10) Office space and equipment will be provided, however, working outside of the office is permitted when approved by your immediate Supervisor.
11) Required to return calls, emails, and texts within 1 business day, unless an exception has been approved by your immediate supervisor.
12) Be kind, professional, ethical, and respectful to all individuals, providers, families, and staff.
13) Follow the LinkAbility HR Manual and TCM Master Contract.
14) Perform other duties as assigned by any member of the Management Team.
Education and Experience:
1) Bachelor’s Degree is required from an accredited college/university.
2) 1 year working with individuals with developmental disabilities.
Physical Demands:
1) Adequate vision and manual dexterity.
2) Ability to bend, sit, stand, stoop, lift, and reach.
Knowledge, Skills and Abilities:
1) Knowledge of persons with intellectual/developmental disabilities.
2) Knowledge of reporting suspicion of potential abuse, neglect, or exploitation and following the protocol according to all applicable regulatory requirements.
3) Skill with the provided electronic equipment and software.
4) Skill with professional work ethic: present a positive image of the agency to the public, uphold confidentiality, and compliance with HIPAA standards.
5) Skill with data-entry, editing, proofreading forms and documents of varying complexity.
6) Skill with basic calculations for budgets along with comparing for accuracy and consistency.
7) Ability to communicate effectively, orally, in writing, and electronically.
8) Ability to assess and evaluate crisis situations, problem solve with other parties involved to resolve conflict in a healthy, positive manner, and consult with your supervisor.
9) Ability to work autonomously and be a self-starter.
10) Ability to be punctual.
Conditions of Employment:
1) Have access to an insured and properly maintained vehicle for work related travel. Must have a valid state driver’s license and auto liability insurance, which meets the requirements set by LinkAbility. Must have an acceptable driving record.
2) Annual review will be completed with your direct supervisor.
3) Ensure Medicaid billing and documentation is true and accurate to real-time.

Salary : $40,000 - $45,000
