Job Summary- Under the direct supervision of the HUMS Program Director, the CHW Post Crisis Specialist (PCS) will work with a varied caseload that will primarily include those individuals that have interfaced with the Crisis Now System in the Matanuska Susitna Borough. This includes people that experience substance use disorders, behavioral and mental health disorders, chronic illness, complex medical, and other circumstances. This position requires development and implementation of care plans, needs assessments, and mutually agreed upon goals in an ongoing collaboration with the client to improve quality of life and health outcomes. This position includes work in the office, the community and in client’s homes.
Responsibilities and Duties
- Will work collaboratively alongside the HUMS Program Director, LINKS staff, Crisis Now partners as well as other community partners to advocate for the needs of the agency, program, and clients.
- Record, input, and update contacts, cases, interactions, assessments, and all data points into Salesforce.
- Attend and participate in monthly multi-disciplinary team meetings.
- Link clients with community agencies, providing warm referrals, and attending appointments with the client as needed.
- Participate in community coalitions that align with the mission of the agency and/or program.
- Assist the Program Director with maintaining relationships with referring agencies.
- Follow up with referrals to ensure access to services, identify barriers to services, and quality of services received.
- Attend staff meetings, training sessions and any other meeting deemed necessary by the Program Director and/or Executive Director.
- Assist in community wide outreach efforts including representation of LINKS and HUMS, and event planning and participation as needed.
- Maintain confidentiality and professionalism at all times.
- Administration duties such as answering phones, scheduling appointments, and directing resources.
- Perform other duties as assigned
Knowledge, Skills, & Abilities
Minimum:
- High school diploma and experience in a medical setting, with case management, and/or social services agency.
- Requires clean driving record, current Alaska drivers license, and auto insurance.
Preferred:
- Degree in Social Work, Human Services, or similar profession or experience advocating for complex clients and performing case management.
- Proficiency with phones and computers and ability to learn and use multiple computer systems.
- Ability to accurately track data, complete and maintain objective documentation
- Considerable knowledge of governmental and non-governmental resources available
- Strong written and verbal communication
- Ability to maintain healthy boundaries
Environment:
This is a full-time position between the hours of 9:00am-5:00pm.
Weekends included as HUMS operates 7 days per week.
Will be required to be on-call 1 week per month from 5:00pm-9:00pm
Mileage reimbursement is available for work related travel.
Eligible for LINKS employee benefits package.
Must be able to pass the State of Alaska, Division of Health Care Services Background Check Program.

Pay: $23.00 - $25.82 per hour
