Position Summary:
The RN Delegating Case Manager will provide nursing case management and delegate RN services to a caseload of individuals with autism and related disabilities in our residential program.
Essential Duties and Responsibilities:
Provides nursing case management and delegating RN services to a caseload of individuals with autism and related disabilities
The RN Delegating Case Manager provides services that include:
o Assessing individuals health needs
o Developing care plans
o Visits homes and supervises direct support professionals in implementing delegated nursing tasks and medication administration.
Serves as liaison with physicians, other healthcare professionals, and family members.
Maintains appropriate nursing documentation including required nursing reviews.
Assesses the medication administration process, including storage, delivery, and other related health care practices at residential homes.
Provides staff training.
Participates as an integral member of each individuals support team.
Must be available for on-call responsibilities, flexible with day/evening hours.
May perform other duties as assigned.
Minimum Qualifications:
DDA-Delegating Nurse experience preferred or must be willing to complete the MBON mandated DDA RN Case Manager/Delegating Nurse/MTTP Trainer Course within 3 months of hire
Valid and active Maryland RN license, BSN, or Bachelors degree in a related field.
Active CPR/AED certification for healthcare professionals.
Two (2) years of experience working in human services and with individuals with developmental disabilities.
Must have a valid drivers MD license and an acceptable driving record as determined by criteria established by the agencys insurance carrier and by agency policy.
Must have available transportation to visit sites.

Physical Demands and Work Environment:
The noise level in the work environment is usually moderate to loud.
Frequently required to use hands or fingers, handle or feel objects, tools, various forms of technology, or controls.
Must be able to stand, walk, sit, reach with hands and arms, climb or balance, and stoop, kneel, crouch, run, or crawl.
Must be able to work under regular, moderate levels of stress.
Must be able to lift and move up to 10 lbs. regularly and occasionally lift or move up to 50 pounds.
Salary : $42
