Little Debbie's provides 24 hour care for adolescents who need this level of care due to complex emotional and behavioral issues. The programs provide stabilization, behavioral support, education around daily living skills, recreational and therapeutic activities, educational linkage and support, case management and coordination of care. Little Debbie's provides a family involved, trauma informed, and strengths-based approach to care within residential communities in the DeKalb area.

POSITION DESCRIPTION

The Director is responsible for overseeing the coordination of children and/or adolescents in placement with a wide range of responsibility for documentation, case coordination and coaching. Because of the life experiences and trauma histories that our residents bring to their placement here, staff must not only provide adequate safety and supervision, but must be actively engaged in the process of building relationships, understanding and solving problems as part of a team, and must offer energy, safety and commitment in their work with our residents through genuine, caring and emphatic engagement. The program support family treatment and involvement. Providing facilitation, supervision, and support for visits and family connections with parents, relatives and other significant people in the life of our youth.

This position provides coordination of and oversight to direct services to residents in the LDSCH Maximum Room Board & Oversight setting. Primary duties include but are not limited to the following:

  • Coordinating Services and Collaboration
  • Make necessary referrals to meet consumer's health, well-being, education, mental health, and employment needs (i.e., Department of Public Health, Department of Family & Children Services, Georgia Department of Labor, employment staffing agencies, local colleges and tech schools, etc.)
  • Facilitate Team Meetings for all residents on caseload with resident as available, residential staff and community stakeholders, including custodians and referral or funding sources as appropriate
  • Coordinate and facilitate quarterly Service Plan reviews as a part of the above Service Team Meetings. When caseworkers, custodians, and referral sources cannot attend the meeting to provide direct input, solicit input via telephone and assure written follow up to provide content of the meeting Collaborate and communicate with other programs in the LDSCH continuum in the provision of services to residents
  • Collaborate and consult with other providers and entities in the provision of best care services to consumers
  • Monitor the provision of services provided
  • Specific services to consumers
  • Attend special activities as needed to support children in care (i.e., graduation, honors, awards, etc.)
  • The Director conducts monthly visits with each program resident
  • Oversee the Coordination of discharge planning and to meet with family members, DFCS and/or DJJ case workers as needed to facilitate discharge plans
  • Facilitate/co-facilitate at least monthly groups and/or house meeting with youth in the home
  • Specific services to program operations
  • Take part in staffing referrals
  • Assist as needed with emergency RBWO emergency intakes and other intakes as needed.
  • Assure that no child will be moved from one placement to another, either internally or externally, without the prior approval of the DFCS and the execution of a new placement agreement
  • Maintain a working office in at least one group home and a work area in the other. Visit each home routinely and act as a second set of eyes to assure compliance with organizational standards.
  • Ensure that all placements, discharges, and transfers are done in partnership with the custodian and/or funding source
  • Review the intake packet and disseminate safety and service plan information from the Intake Department to residential staff
  • Facilitate Utilization Reviews for assigned residents
  • Assure contact with the youth’s custodian and/or referral source as soon as possible after the youth admission to assure an understanding of the program, meeting times for service planning, etc. Assure all documentation required from the custodian or referring agency not available at intake is obtained
  • Assure compliance with the custodian agency’s case plan
  • Engage in on-call support to staff and house coverage
  • Work in collaboration with the house staff and House Supervisor to ensure needs of youth are met
  • Documentation Requirement

  • Works on other duties, tasks and projects as assigned
  • Ensures agency compliance with National Council on Accreditation and other mandatory regulations/standards
  • Lives and works by LDSCH Values and Code of Ethics
  • Follow mandated reporting requirements of GA Code
  • Support and maintain an environment of emotional and physical safety for consumers, staff and other stakeholders at all times
  • Demonstrate customer service
  • Promote positive image of LDSCH programs at all times
  • Work as a contributing and collaborative team member
  • Respond to consumer and peer needs in a timely fashion
  • Answer telephone calls and emails accordingly
  • Respond to requests for information from stakeholders in a timely fashion
  • Maintain Personal Information and Training
  • Attend and participate in staff meetings as scheduled
  • Attend and participate in required trainings as scheduled
  • Maintain required certifications
  • Review, understand and responsible to program policies
  • Provide documents in a timely fashion to Human Resources
  • Report changes in status to HR as required by policy

QUALIFICATIONS/COMPETENCIES

The Director shall be at least 25 years of age and preferably possess a bachelor’s degree, preferably a master's degree, in social work, psychology, education counseling or related social service field plus two years of relevant health care, behavioral health or community-based experience working with women, families, and/or children and be supervised by an individual with a master’s degree in one of the above named fields. Must be proficient with office productivity software such as Outlook, Word and Excel. Must have experience using databases. Must be able to produce a business writing sample.

SCHEDULE

Work Hours (an overall general statement):

The work schedule requires flexibility to meet the needs of the operations, residents and homes. Director will weekends as needed. The Director is to consider LDSCH, Inc. as their primary employer.

The Director is scheduled a minimum of 80 hours bi-weekly.
