Summary Job Description:
Principle Responsibilities:
- Apply for birth certificates from ACS
- Direct communication with adoption lawyers
- Ensures that the adoption petitions are signed, notarized, and sent to the attorney for filing
- Redact records when warranted before sending them out to legal (cases involving lawsuits).
- Process SCRs for adoptive parents

Prepare packet for home study which consists of:
- A copy of the original home study, fingerprints, SCRs & marital documents of adoptive parents.
- Initiate the TPR folder
- Create template for the adoption homestudy
- PPG goal change in the system to adoption & KinGap
- Enters the adoption milestones in the intranet & Connections
- Maintain the adoption electronic records by uploading adoption milestone dates (TPR dates, hearing dates, adjournment dates)
- Tracks # TPR filed for the fiscal year
- Tracks cases that TPRs are not required for (for example, a decease parent/no TPR is necessary)
- Uploads TPRs documents, Court documents, petitions and emails onto the intranet
- Enters legally freed dates in the system
- Tracks & sends the final/raised seal of the adoption agreement.
- Sends packet to ACS after adoption is finalized
- Enter the M921 form to indicate when adoption was completed; sent same day of the adoption.
KinGap Liaison
- Trained in the KinGap system to enter and updates the KinGap cases
- Follow-up with staff to obtain updates on the cases
- Prepare monthly report to ACS/Kin Gap liaison to report on Kin Gap/legal status
- Files the Kin Gap package to send to ACS once guardianship is awarded
- Apply for Medicaid for all youth who are discharged after Kin Gap guardianship is awarded
Post Adoption
- Responds to inquiries from adult adoptees looking for information about biological parents
- Sends package to adoptive families about post adoption services
- Tracks all incoming calls/requests for records.
Minimum Requirements:
- A Bachelor’s Degree or higher.
- Previous office experience, including experience with Word and Excel.
- Previous experience in child welfare.
