Job Description

Job Title : LPN - Part Time Evenings

Job Specialty : Long-Term Care (LTC) LPN

Job Duration : 13 Weeks

Shift : Part-Time, 3x8-hour evening shifts alternating weekly

Guaranteed Hours : N / A

Experience : Minimum 1 Year in an LPN Position in an LTC Facility

License : NY LPN Licensure

Certifications : BLS

Must-Have :

  • Experience in providing care to geriatric patients
  • Ability to demonstrate knowledge and skills necessary for patient care based on their age and needs

Job Description :

  • Provide direct and indirect skilled nursing care for assigned residents, following defined policies and procedures under the supervision of a Registered Nurse.
  • Maintain a collaborative relationship with other nursing staff, physicians, and ancillary staff.
  • Maintain knowledge and clinical expertise through continuing education and in-service programs.
  • Collect data relevant to residents' physical and psychosocial condition on an ongoing basis and communicate findings to the RN and other Interdisciplinary Team members.
  • Contribute to creating a developmentally age-appropriate and culturally relevant plan of resident care, reflecting nursing diagnosis and interventions to achieve desired outcomes and involving the resident / family and healthcare team.
  • Recommend revisions to the plan of care after evaluating resident outcomes and indicate the need for referrals to ensure all care aspects are addressed.
  • Collaborate with the resident / family and Interdisciplinary Team members within the nursing practice scope, ensuring customer satisfaction.
  • Maintain current knowledge and competency in Nursing Practice, participating in staff mentoring / orientation.
  • Consider factors related to safety, cost, and impact in planning and delivering care.
  • Under the direction of the RN, provide leadership in the professional practice setting.
  • Demonstrate knowledge and competency, with occupational exposure to blood-borne pathogens reasonably anticipated. Required tasks involve potential contact with blood, body fluids, or tissues.
