Job Description

Job Title : Licensed Practical Nurse (LPN)

Job Specialty : Long-Term Care (LTC)

Job Duration : 14 weeks

Shift : Full-Time Evenings, 5x8 hours

Guaranteed Hours : Not specified

Experience : Minimum of 1 year as an LPN in a Long-Term Care Facility

License : New York LPN Licensure

Certifications : Basic Life Support (BLS)

Must-Have :

Experience in providing care to adult and geriatric populations

Job Description :

Demonstrates knowledge and skills necessary to provide care appropriate to the age of patients served.

Collects data relevant to the resident's physical and psychosocial condition on an ongoing basis and communicates findings to the Registered Nurse and other members of the Interdisciplinary Team.

Contributes to the creation of a developmentally age-appropriate and culturally relevant plan of resident care, reflecting nursing diagnosis and interventions to achieve desired outcomes.

Recommends revisions to the care plan after evaluating resident outcomes and indicates the need for referrals to ensure comprehensive care.

Maintains a collaborative relationship with other nursing staff, physicians, and ancillary staff.

Participates in continuing education and in-service programs to maintain current knowledge and competency.

Assures a healthy and safe environment while promoting customer satisfaction.

Considers factors related to safety, cost, and impact in planning and delivering care.

Provides leadership in the professional practice setting under the direction of the Registered Nurse.

Occupational exposure to bloodborne pathogens is reasonably anticipated, requiring caution and adherence to safety protocols.
