Job Title : LPN Part Time - Evenings

Job Specialty : LTC LPN

Job Duration : 13 weeks

Shift : Part Time, 3x8 alternating with 2x8 every other week; Every other weekend Fri & Sat night

Guaranteed Hours : Not specified

Experience : 1 year experience as LPN in a Long-Term Care (LTC) Facility

License : NY LPN Licensure

Certifications : BLS

Must-Have :

  • Ability to provide care to geriatric residents

Job Description :

  • Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse.
  • Collect and communicate data relevant to residents' physical and psychosocial conditions to the Interdisciplinary Team.
  • Contribute to the creation of a developmentally, age-appropriate, and culturally relevant plan of care for residents.
  • Recommend revisions to care plans after evaluating resident outcomes and collaborate within the scope of nursing practice.
  • Maintain current knowledge and competency in nursing practice through continuing education and in-service programs.
  • Participate in mentoring and orientation of staff, and consider factors related to safety, cost, and impact in care delivery.
  • Occupational exposure to blood-borne pathogens is reasonably anticipated, with tasks potentially involving contact with blood, body fluids, or tissues.
