JOB SUMMARY:
The Nurse Case Manager at our Rural Health Clinic will be responsible for providing professional nursing care and comprehensive case management services to patients. This role involves working collaboratively with healthcare team members to address the physical, psychosocial, and educational needs of patients and their families. The Nurse Case Manager will focus on managing chronic health conditions, optimizing patient outcomes, and improving the quality of care in our family and internal medicine departments.

ESSENTIAL FUNCTIONS, DUTIES AND RESPONSIBILITIES:
- Patient Care Coordination: Assess, plan, implement, and evaluate individualized care plans for patients. Coordinate care across different services and providers, ensuring seamless patient transitions between inpatient, outpatient, and home care. Facilitate communication between patients, families, and healthcare providers to ensure understanding and adherence to care plans.
- Chronic Disease Management: Conduct thorough assessments of patients with chronic health conditions, including medical history, current symptoms, medication management, and psychosocial factors. Develop individualized care plans in collaboration with patients, caregivers, and healthcare providers, outlining goals, interventions, and timelines for managing chronic conditions effectively. Educate patients and their families about disease management, medication adherence, and lifestyle modifications.
- Resource Management: Connect patients with community resources and support services to enhance their quality of life and care. Assist patients in accessing financial assistance programs, transportation, and other necessary services.
- Patient Education and Support: Provide health education and counseling to patients and their families on preventive care, self-care strategies, and healthy lifestyle choices. Support patients in understanding their diagnoses, treatment options, and the implications of their health conditions.
- Quality Improvement: Participate in quality improvement initiatives aimed at enhancing patient care and clinic operations.
- Documentation and Compliance: Maintain accurate and timely documentation in patients' medical records. Ensure compliance with clinic policies, state and federal regulations, and best practice standards.
- Multidisciplinary Collaboration: Work closely with physicians, nurses, social workers, and other healthcare professionals to deliver comprehensive care.
ADDITIONAL RESPONSIBILITIES:
- Clinical Nursing Duties: Prepare equipment and assist physicians during treatment, examination, and testing of patients. Record patients weight, blood pressure, pulse, height, and other vital signs. Observe, record, and report patients condition and/or response to treatments to physicians. Authorize drug refills as directed by physicians or standing orders. Triage patients to the appropriate setting if there is a need to be seen. Arrange for patient testing and admissions.
- Attend at least 80% of nursing staff meetings and complete all required educational competencies and mandatory in-service classes.
- Support and model individual behavior consistent with the Standards of Excellence and the mission, vision, and values of the clinic.
QUALIFICATIONS (Required):
- Bachelor of Science in Nursing (BSN) required; Master of Science in Nursing (MSN) preferred
- Current Registered Nurse (RN) license in the state.
- Current CPR certification.
ADDITIONAL DESIRABLE QUALIFICATIONS:
- Strong commitment to patient-centered care and community health.
- Minimum of 2-3 years of nursing experience, with at least 1 year in case management or a related field.
- Experience in family medicine, internal medicine, or primary care settings is highly desirable.
- Strong clinical assessment and care planning skills.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work independently and as part of a team.
- Proficiency in electronic health records (EHR) systems and other healthcare technology.
- Knowledge of community resources and healthcare systems.
- Experience working in rural health settings is a plus.
