What are the responsibilities and job description for the Medical Case Manager for Health Works (DCFS) position at Logan County Department of Public Health?
The Logan County Department of Public Health is accepting applications for a candidate who meets one of the following qualifications:
- Be a registered professional nurse licensed by the state of Illinois and
- Associates or Bachelor of Science in Nursing degree from an accredited program
- At least 2 years clinical experience or experience in pediatrics preferred.
- Be a clinical social worker licensed pursuant to Section 9 or social worker licensed pursuant to Section 9A of the Clinical Social Work and Social Work Practice Act and
- One year of experience in providing services to families with young children preferred.
- A Master’s degree or Baccalaureate degree in a behavioral science, social science, or health-related area; or a Baccalaureate degree in any other area and one-year experience in child, family or community services; or an Associate degree and two years’ experience in child, family or community services. Case managers meeting only this qualification must be supervised by a case manager meeting the requirements of (1) or (2) above until they have a total of two years of supervised case management experience.
- This position requires excellent oral and written communication skills; self direction with the ability to work with little supervision; good organizational skills; ability to meet occasional daytime travel requirements; flexibility and cooperation in fulfilling role obligation. Familiarity with Evolve software program may be helpful but is not required.
- Duties include the provision of interim case management for all children in the legal care and custody of the Illinois Department of Children and Family Services and who are in the substitute care in the areas to be served by the Logan County Department of Public Health as Lead Agency. This includes participant education, facilitation of access to services, coordination with service agencies, follow-up on services delivered, assistance with scheduling and case management assessments to determine medical, psychosocial, and environmental risks and needs. Due to the caseloads, some filing may be required as part of the day-to-day routine.
- Other duties may be assigned as needed.
