Responsibilities
- Assess clients for initial transplant suitability and re-evaluate candidates as directed
- Counsel clients and facilitate group therapy sessions to address emotional and psychological needs
- Provide diagnostic evaluation, individual, family, and group therapeutic counseling, and resource referral services to patients and their families
- Collaborate with healthcare providers to ensure seamless care coordination and transition planning
- Refer clients to community resources to enhance compliance with complex medical regimens
Requirements
To be successful in this role, you will need:
- A Master's degree in social work (MSW)
- A minimum of one year of experience in a clinical setting, preferably in a hospital or healthcare environment
- Excellent communication, interpersonal, and problem-solving skills
- The ability to work effectively in a fast-paced, dynamic environment
