Position Summary
Essential Functions and Responsibilities of the Job
- Timely and accurate response to the discharge planning referrals.
- Follows standardized processes and decision trees for referrals to community clinical and social resources.
- Charting in the medical record systems, per standard work.
- Active engagement of the patient and patient's representative in the development of the discharge plan.
- Performs other duties as assigned.

Qualifications/Work Experience:
- Previous discharge planning experience and/or community resource development expertise is preferred.
Education/Licensure/Certification:
- Prefer LVN or EMT or higher degree in a healthcare related field.
Salary : $31 - $45
