ORGANIZATION OVERVIEW :

Lorain County Health & Dentistry (LCH&D) is a private, nonprofit, charitable organization dedicated to delivering the highest quality primary and preventive health care to underserved persons of all ages throughout Lorain County, OH and beyond. Founded in 2002, LCH&D first operated as a Federally Qualified Health Center (FQHC) Look-Alike until designated by the federal Health Resources and Services Administration (HRSA) as a funded FQHC in early 2009.

The largest safety-net provider in Lorain County, LCH&D operates six community health centers in Lorain, Elyria, Oberlin and North Ridgeville and provides adult / family medicine, pediatrics, general and specialty dentistry, podiatry, optometry, pharmacy and integrated behavioral health, including Medication for Opioid Use Disorder. A focused vision, solid mission, effective strategic planning, excellent leadership and management, teamwork, and perseverance have enabled LCH&D to grow significantly over the years. As a result, LCH&D is a well-known and respected organization locally and at the state and federal levels.

Since its founding, LCH&D has provided over one million doctor and dentist visits. In 2023, LCH&D treated 18,575 Unduplicated Patients in 63,222 medical and dental visits. "Quality" is highly valued at LCH&D, and the health center is dedicated to providing the best clinical care and customer service possible. Today, LCH&D places in the Top 10% for best overall clinical quality measure performance among all health centers nationwide. As such, HRSA recently awarded LCH&D the coveted 2024 Health Center Quality Leader Gold Badge. LCH&D has more than 120 providers and staff and an $18 million annual operating budget. In addition to providing excellent clinical care, the organization is well positioned both operationally and financially.

Hours : Full-time, Monday-Friday 8 : 30am - 5 : 00pm

PRIMARY PURPOSE

The Population Health Manager works in a collaborative role as a leader in a multidisciplinary care team in the ambulatory care setting based on the organization's patient centered medical home model. The Population Health Manager's goal is to improve health outcomes of a group or groups of individuals while recognizing patterns and responding with intervention and policy. The individual must process and manage large quantities of data to monitor key performance indicators and research any barriers to the strategic direction of the population health program. If a barrier is identified, this position coordinates care and interventions based on the member's healthcare needs and goals and follows up with the appropriate supports to attain those goals. The Population Health Manager uses tools to target outreach and coordinate care in response to various indicators including, but not limited to, Managed Care Organization (MCO) reporting, MCO portals, AZARA DRVS reporting, MCO attribution risk stratification. The Population Health Manager facilitates steps to achieve optimal outcomes and as such plays a vital role in coordinating patient care in accordance with LCH&D's philosophy of "partnership" with patients. While achieving optimal clinical outcomes, the gap closures and excellent clinical outcomes will maximize Value Based Care incentives and rewards.

EDUCATION AND CERTIFICATION REQUIREMENTS

  • Bachelor's degree in Public Health / Healthcare management / related field
  • Will consider RN with active Ohio license with experience in Population Health Management

SKILL AND EXPERIENCE REQUIREMENTS

  • Minimum of Two (2) years experience working in a Community Health Center with working knowledge of value-based care, managed care, population health management, quality / process improvement or project management.
  • Must have knowledge of care management and population health principles.
  • Motivational Interviewing Training preferred.
  • Strong computer skills with proficiencies in Outlook, Word, PowerPoint, and Excel.
  • Must have strong organizational skills.
  • Ability to work independently with minimal supervision.
  • Ability to maintain a calm and professional demeanor and communicate enthusiastically with patients and staff
  • Ability to be responsive in working with a culturally-diverse patient population.
  • Must have excellent communication skills with the ability to master a therapeutic communication approach.
  • Ability to independently coordinate multiple tasks.
  • Ability to work cooperatively with others.
  • Must have valid driver's license and satisfactory driving record.
  • Bilingual Spanish speaking is a plus.
  • PHYSICAL DEMANDS

  • Extensive use of computer and telephone.
  • Some lifting of supplies and equipment up to 50 lbs.
  • 25% walking or standing, 75% sitting at a desk.
  • Category I re : exposure to blood, bodily fluids, and communicable diseases.
  • PUBLIC AND PROFESSIONAL ACTIVITES

  • Travel between sites may be required.
  • Attend relevant professional activities such as organization sponsored events as needed.

  • ESSENTIAL FUNCTIONS

    Using AZARA DRVS and other data analytic tools :

  • Identifies gaps in patient care and develop response actions
  • Participates in and oversees population outreach efforts using patient centered approach
  • Motivational interviewing

  • Self Management strategies
  • Functional Lifestyle goals
  • Assist with steps to achieving provider driven treatment goals
  • Identifying and assisting to overcome barriers in patient care
  • Monitor and manage ED and Hospital Utilization through the TOC module
  • Reporting of ED and Hospital utilization at CQI

  • Identify high utilization and develop strategic plan for outreach
  • Organize and contribute to ED and Hospital Utilization PDSA with partnering hospital systems when necessary
  • Collaborate with LCH&D Care Management teams and / or with the MCO Care Management team for assistance when assistance is needed beyond what LCH&D can provide in an outpatient setting
  • Working collaboratively with each Managed Care Organization and participating payers with value based incentives :
  • Maintain knowledge and understanding of each incentive structure

  • Ohio Department of Medicaid program point of contact
  • Monthly progress performance meetings
  • Portal entry for review of PCOR and other reports
  • Monthly attribution review for enrollment and risk stratification
  • Gap closures
  • Ohio Comprehensive Primary Care Program adherence
  • Maximize Value Based Care reimbursements and other program incentives
  • Working knowledge of each payer plan and incentives

  • Collaboration with MCO and assigned leads
  • Ensure the proper use of CPT codes to maximize adherence through claims
  • Ensure timely supplemental data is submitted, when allowed, to qualify for incentives
  • Collaborates and participates in the Lorain County Public Health Community Health Improvement Plan (CHIP) as an active member of the team and reporting activities to the Continuous Quality Improvement Committee quarterly.
  • Working with the Continuous Quality Improvement Committee, the Population Health Manager will have a full understanding of the Uniform Data Systems reporting and will assist to obtain the highest clinical outcomes possible by way of gap closure and patient outreach and intervention.
  • Will assist the Quality and Clinical teams with input to improve patient health literacy, awareness and understanding of the effects of chronic disease and the importance of screening, detection and intervention with the ultimate goal of improving health center and community outcomes.
  • Function in accordance to Quality related programs including NCQA / PCMH, CPC, and CQI plan
  • Demonstrate competence in all areas of assigned job responsibilities.
  • Must maintain accurate records.
  • Comply with HIPAA and CFR 42 part 2 guidelines as all times
