Now Hiring: MDS Coordinator (RN)
We are seeking a skilled and experienced MDS Coordinator to join our clinical leadership team. This is an excellent opportunity to contribute to resident care planning and compliance processes while working with a supportive and experienced team.

Key Responsibilities:
Coordinate and complete all components of the Resident Assessment Instrument (MDS, CAAs, and Care Plans) in compliance with federal and state regulations.
Accurately enter ICD-10 diagnosis codes upon admission, re-admission, or hospital transfer.
Collaborate with the Interdisciplinary Team to ensure timely, complete, and accurate assessments.
Lead and participate in daily PPS huddles, weekly Medicare meetings, and monthly close-outs.
Conduct resident interviews and medical record reviews to ensure comprehensive and person-centered documentation.
Ensure all MDS-related documentation is signed, dated, and compliant.
Serve as Manager On Duty one weekend per month, with a weekday take-back day.
Partner with hospitals and external providers to collect accurate and complete information during transitions of care.
Support quality assurance processes, including chart audits and direct care observations.
Qualifications:
Current Registered Nurse (RN) license in the state of North Carolina (NC) – Required
Minimum 3 years of experience as an MDS Coordinator – Required
Active CPR certification
Prior experience in Long-Term Care
Strong knowledge of nursing practices, care planning, and regulatory compliance
Excellent communication and team collaboration skills
High level of professionalism and personal integrity
