Summary Objective
The purpose of this position is to plan and coordinate community health programs with health and human service programs. Provides technical assistance, consultation, education and instruction to staff and the community. Develops and evaluates program procedures and recommends modifications.
This class works under general supervision, independently developing work methods and sequences.
Examples of Duties
The essential functions listed below are those that represent the majority of the time spent working in this class. Management may assign additional functions related to the type of work of the class as necessary.
- Assists in budget preparation and monitors and controls expenditures.
- Participates in quality assurance activities.
- Develops and maintains a data collection system to monitor and report program progress toward achievement of project goals and objectives.
- Monitors service provider performance for compliance with program standards.
- Evaluates service provider programs for effectiveness, and recommends improvements to enhance efficiency, services and performance.
- Conducts training sessions, lectures and seminars on health-related issues.
- Attends meetings to discuss and provide information on goals, needs and services with health and human service providers, community organizations, and advisory councils.
- Completes special projects, and prepares reports.
- Performs related work as assigned.
Minimum Requirements
- Bachelor’s Degree in Business or Public Administration, Education or related discipline.
- Two (2) years of nursing, primary care, community or public health, health education, or social service experience or an equivalent combination of education, training and experience.
- Must provide one's own source of transportation.
- Must possess and maintain a valid state driver’s license with an acceptable driving history.
Physical demands refer to the requirements for physical exertion and coordination of limb and body movement.
- Performs sedentary work that involves walking or standing some of the time and involves exerting up to 10 pounds of force on a regular and recurring basis or sustained keyboard operations.
