What are the responsibilities and job description for the Social Services Case Manager - Lourdes Counseling Center - Adult Unit - PRN - Rotating position at Lourdes Health?
The Social Services Case Manager is a member of the multi-disciplinary treatment team assuming responsibility for: assessing the psycho-social needs and resources of the acute hospitalized mentally ill consumer; coordinating and implementing psycho-social services; assessing the discharge needs and resources of the consumer and their significant relations; assisting the consumer and/or their significant relations to obtain necessary services and resources. A critical attribute required of this position is exceptional interpersonal customer services consisting of an outgoing positive attitude, warm friendly joyful demeanor, and the utmost care and reverence for our guests, patients, families, customers and colleagues.
Rate: $20.64 - $29.07 Hourly

EDUCATION
- Education: Masters in Social Work (MSW), or a Masters in related field required.
- Population served: Knowledge and skills necessary to provide care appropriate to the population served.
- Experience: Three (3) years’ experience in a social service agency; mental health setting preferred.
- Certification Required: Agency Affiliated Counselor Registration or Licensed Mental Health Counselor or Social Worker in the State of Washington.
- Other: Bilingual and/or English/Spanish-Speaking preferred. Ability to work with culturally diverse population
