Job Title: Social Worker (Case Manager)

Our agency is committed to support and empower youth and adults with skills they need to live healthy lives. We assist youth to be better prepared for successful transition into independent adulthood and self-sufficiency. Clients receive services that include stable housing, education and employment training opportunities. The Case Manager works under the direction of the Program Manager.

Case Manager Functions:

• Meet client regularly, conduct intake assessments, and create individual case plans for caseload.
• Provide individualized case management and counseling for clients
• Provide crisis intervention as needed within a program
• Coordinate case management services with other staff to ensure the quality of service
• Prepare for and participate in weekly clinical supervision regarding cases with LCSW Consultant.
• Organize and maintain participant’s files.
• Weekly case review and monthly reports to Program Manager.
• Attend weekly staff meetings with additional training and conferences.
• Facilitates social support groups.
• Other duties as assigned.

Social Worker Qualifications

• Bachelor’s degree in social services or related field. Areas of education with an emphasis in counseling Social work or social welfare.
• Excellent writing and verbal communication skills; computer literacy (PC).
• Demonstrated group work experience including life skills and job preparation instruction.
• Commitment to working with disadvantaged individuals from diverse backgrounds.
• At least two years' experience working with at-risk and disadvantaged individuals in crisis intervention and case management.
• Ability to manage multiple responsibilities and follow-up with participants in a timely manner.
• Must pass criminal and driving background checks
• Class B license Driver’s License and auto insurance.

Physical Requirements:

• Stationary position 40% of the time.
• Operates office machineries such as fax, copy machine, phone, and computer.
• Occasionally bend, reach, kneel, ascend/descend stairs, and move inside and outside the office and buildings. Frequently moves objects weighing up to 25 pounds.
• Required to have visual acuity to perform the following tasks: operate a motor vehicle, viewing a computer terminal, reading and writing, assess participants, conduct unit inspections, etc.

Compensation:

Depending on education and experience.
