What are the responsibilities and job description for the EIS Peer Adherence Coach position at Low Country Health Care System?
Hours: Monday – Friday 8:00am-5:00pm -(40 hours/week). Other hours and/or days as needed for program activities.
Qualifications: High school diploma or the equivalent. Must be a client receiving EIS services and a Person Living with HIV. It is imperative that this person is adherent to therapy and not currently using or abusing substances. Personal history and experience with Ryan White services. Strong communication and interpersonal skills. Compassion, empathy, and the ability to build rapport with patients. Knowledge of HIV treatment and care guidelines. Excellent organizational and documentation abilities. Commitment to maintaining patient confidentiality. Ability to work effectively as part of a healthcare team. Cultural sensitivity and the ability to work with diverse populations. Must be able to travel both in state and to occasional national meetings. Required to have direct client contact in order to provide program activities. Must possess excellent communication skills and be committed to helping others. Will be required to travel to Allendale, Barnwell, Bamberg, Aiken and surrounding counties.
Job duties include but are not limited to:
Patient Education:
• Provide comprehensive education to patients about HIV, its treatment, and the importance of medication adherence.• Offer clear explanations of treatment regimens, potential side effects, and ways to manage them with the support of a healthcare professional.
Adherence Support:
• Provide Peer Adherence services to positive clients living with HIV.
• Develop and implement strategies to monitor and improve patients' adherence to prescribed HIV medications.
• Collaborate with healthcare providers to address any adherence barriers or challenges.
Treatment Plan Coordination:
• Work closely with the healthcare team to ensure patients have access to the most appropriate and up-to-date treatment options.
• Assist in creating individualized plans that assist with patients’ comorbidities, medical histories, needs, and preferences.
Follow-Up and Monitoring:
• Maintain a schedule of regular follow-up appointments with patients to assess their progress and adherence.
• Maintain detailed and accurate documentation of patient interactions with the clinic and/or interventions provided by the agency (i.e. Support Groups, Lunch and Learns, etc.)
Peer Mentorship:
• Serve as a peer mentor to establish a supportive community among patients/people living with HIV.
• Share personal experiences and success stories to inspire and motivate patients to maintain adherence.
Professional Development:
• Stay current with the latest developments in HIV treatment/care guidelines, and emerging therapies.
• Participate in relevant training sessions, workshops, and conferences to enhance knowledge and skills.
Confidentiality and Ethics:
• Adhere to strict confidentiality standards regarding patient information.
• Conduct all interactions and interventions in an ethical, respectful, and professional manner.
Crisis Intervention:
• Be prepared to provide immediate support, guidance, and referrals in case of medication- related emergencies, side effects, or mental health crises.
Community Engagement:
• Engage with the local HIV community and other organizational partners to raise awareness about HIV prevention, care, treatment, and support.
• Promote testing and early diagnosis initiatives to reduce the spread of HIV and increase viral suppression
Other Duties as Assigned
- Coordinate Peer Support Groups
- Keep current clients up to date with available services

Applicants with “previous like skills” may not be given the same weight and consideration as “previous specific same skills”: i.e., For a Medical Receptionist position, “Previous clerical skills” may not be preferred over “Previous Medical Clerical Skills”.
