Job Description
We are seeking a dedicated Nurse Practitioner to join our team as Primary-Care Providers.
Requirements and Qualifications
- Experience in Family Medicine, Internal Medicine, or Geriatrics is preferred but not required.
- New graduates are welcome to apply.
- Candidates must hold a valid and unrestricted medical license in Michigan.
- Candidates must have a valid DEA license in the state of Michigan.
- Candidates must have completed a U.S. certification program.
Job Structure
- This position requires an in-person presence.
- Providers will commute directly between their homes and facilities.
- Nurse Practitioners work in collaboration with an LTC Collaborating Physician.
- Nurse Practitioners are responsible for providing day-to-day care to their patients.
- After-Hours on-call duties are shared among all LTC Providers.

- We offer a Productivity Based Compensation Model without any caps.
- A $10,000 SIGN ON BONUS is available.
- 15 PTO days are provided annually.
- An annual CME allowance of $2,500 plus 3 PTO days for CME attendance is included.

- A monthly phone stipend is offered.
