This opportunity is for LPNs who have held a position at Luthercare for at least 90 days before applying for the mentor role.
We’re currently seeking dedicated Personal Care LPN Mentors for Personal Care at Luther Acres, a Luthercare community in Lititz, PA. We offer full-time, non-exempt positions (7A-3P, 3P-11P, 11P-7A) Monday-Friday, weekends as needed.

WHAT YOU DO:
As a Personal Care LPN Mentor at Luther Acres, you provide mentorship, support, and training to new employees embarking on their journeys with Luthercare. Your leadership will play an important role in fostering a collaborative environment within our nursing team. You’ll do this in several ways, including:
- Assist new employees in acquiring the experiences and skills needed to be successful in their new positions. Support new employees through the orientation process and beyond, available to respond to orientees’ needs and present a professional attitude, performance, and supportive manner. Introduce new employees to coworkers and make them feel welcome.
- Provide advanced medical support and direction to the care center and direction to staff and campus residents as needed.
- Assist with assessments & RASP (Resident Assessment Support Plan) process. Also assist with admission, discharge and transfer process.
- Assist residents in completing basic functions to include but not limited to: bathing, grooming, physical/oral hygiene, shaving, hair grooming/shampooing, dressing, and assist residents at mealtimes. Assist and supervise residents with medications.
- Provide functional supervision to personal care staff. Assist supervisor with training and in-service programs.
- Receive verbal orders from physicians including lab work, x-rays, holter monitor requests, etc. Provide follow-up with appropriate documentation. Verify medication orders with physician upon resident admission or discharge from hospital. Assist with resident health assessments. Supervise and assist residents with medications, including insulin injections and blood sugar checks. Performs nursing duties following licensure scope of practice.
- May provide assistance with general housekeeping tasks including but not limited to: the changing of linens, proper care of clothing, maintaining general appearance, dust and polish furniture, clean wash basins, tubs and commodes, and emptying wastepaper baskets, and washing floors. May assist residents with use of personal washer and dryer.
- Observe residents and report changes in resident condition or symptoms of illness to supervisor. Obtain and monitor vital signs and assist with emergency situations and ambulation.
- Encourage residents to remain active and attend social functions and programs. Assist with the development and carry-out of activities and programs on a routine basis under the leadership of the activity staff.

Luther Acres – located in the charming, historic town of Lititz – is a Continuing Care Retirement Community with a long history of providing excellent care and services. It is part of Luthercare, which is a not-for-profit, faith-based, and mission-driven organization. Luthercare provides a network of services through its full continuum of care with two communities in Lancaster County. Luther Acres is home to nearly 600 residents living across its independent living cottages and apartments, personal care apartments, and skilled nursing healthcare center.
QUALIFICATIONS/REQUIREMENTS:
- Current & valid PA licensure as a Licensed Practical Nurse.
- Successful completion of the PA Department of Human Services (DHS) Direct Care Staff Person Training Course/Test.
- Individuals in this position must have obtained age twenty-one (21) prior to the first day of employment or transfer into position.
- Current First Aid and CPR Certification, or willingness to be trained by Luthercare during employment.
- Review portions of the PA DHS Medication Assistance Training program as assigned and stay current with regulation changes as necessary.
PREFERRED QUALIFICATIONS/REQUIREMENTS:
- Three (3) years' experience providing basic personal care in a long-term care setting or healthcare center.
- Valid PA Driver’s License.
- Basic computer skills and ability to utilize electronic health record processes
