Summary
We are seeking a Qualified Intellectual Disabilities Professional to join our team at Luther Home of Mercy. This role is essential in providing compassionate care and support to individuals with intellectual disabilities, ensuring they receive the necessary assistance in their daily lives. The position is based at our campus in Williston, Ohio.
This position will be responsible for developing and monitoring service and Habilitation plans for persons with Intellectual and Developmental Disabilities and providing overall leadership to residential supervision and Direct Support staff; along with the following:
ESSENTIAL RESPONSIBILITIES:
1. Provides ultimate direction and support to the Cottage supervisor and Direct Care Staff to ensure that resident’s health safety and programming needs are met. Leads the Cottage Leadership Team meetings and responsibilities. Serves as primary advocate for the residents on caseload and ensures that monitoring functions are completed appropriately.
2. Coordinates the development and implementation of the Individuals Habilitation Plan (IHP) for each individual in the cottage. Writes initial IHP within the first 30days and reviews IHP process with 30-90-day review system and also completes necessary assessments and documents.
3. Develops necessary assessments and plans as required by procedures and standards set forth by Medicaid and other standard setting authorities which include but not limited to annual comprehensive functional assessment, formal behavior management programs/guidelines, physical health and support plans, right modification forms. Provide appropriate training to staff regarding the above plans and procedures related to them.
4. Assesses, develops and implements formal behavior management programs and/or guidelines for each resident as needed, and serves as a resource in the organization’s approaches to behavior support/crisis prevention to other staff members.
5. Facilitates various interdisciplinary team meetings to resolve concerns that arise in the ongoing evaluation, monitoring and implementation of the IHP plans; which include admission meetings, initial IHP, annual IHP, monthly/quarterly reviews, special team meetings and discharge meetings to coordinate comprehensive delivery of services for appointed caseload. Provides input and makes recommendations for policy development. Responsible for documentation and recording of minutes at various meetings.
6. Works cooperatively with nursing personnel to prepare information relating to and attend Medical Reviews, Dental appointments and other appointments when there is a serious health concern for an individual.
7. Meet with appropriate staff and departments to solve problems and provide technical assistance.
8. Facilitates and participates in necessary meetings regarding hospitalizations and Major Unusual incidents to respond to changes in resident needs and services as well as creating recommendations appropriate to the situation.
9. Initiate Unusual Incident follow-up as needed, assist with Incident investigations and participate in incident trend analysis.
10. Coordinates program services including but not limited to speech, medical, pharmacy, social services, psychology vocational, PT, OT, behavior management in addition to services with outside agencies.
11. Develops and monitors the design and implementation of each residents daily active treatment schedule.
12. Performs supervisory functions for assigned staff, including, but not limited to, personnel interviewing and selection, payroll and scheduling functions, performance evaluations and corrective action as required.
13. Performs necessary reviews and checklists within the building to assure activities, programs and procedures are being carried out and appropriately monitored.
14. Maintains appropriate communication with residents’ families and/or responsible parties regarding resident needs and capabilities.
15. Maintain current knowledge in the field of DD and attend various in-services and workshops that are job related.

17. Assist Program Service Department leadership in other related duties as specified.
Salary starting at $25.00 per hour. Position is also overtime eligible.
Requirements
- Bachelor’s Degree in Special Education, Psychology, Social Services or related field
- Strong background in patient care or case management.
- Excellent communication and interpersonal skills.
- Ability to manage conflicts effectively and compassionately.
- At least one-year experience working directly with persons with disabilities.
- Supervisory experience preferred.
