Position Overview :
We are seeking an experienced and detail-oriented MDS Coordinator to join our dynamic team. The MDS Coordinator is responsible for coordinating and overseeing the completion of the Minimum Data Set (MDS) assessments, ensuring compliance with federal and state regulations while supporting optimal care planning and reimbursement processes. We are looking for an RN for this position.
Key Responsibilities :
- Complete and submit accurate MDS assessments in accordance with CMS guidelines and facility protocols.
- Coordinate the RAI (Resident Assessment Instrument) process to ensure timely and accurate assessments.
- Collaborate with interdisciplinary team members to develop individualized care plans for residents.
- Monitor changes in residents’ conditions and update care plans as needed.
- Ensure compliance with state and federal regulations, including Medicare and Medicaid guidelines.
- Provide education and training to staff on MDS processes and documentation requirements.
- Audit resident records to ensure accuracy and completeness of documentation.
- Participate in care plan meetings and quality improvement initiatives.
