RESPONSIBILITIES:
● Conduct all phases of client health and outreach (engagement, assessing strengths, goal planning, safety planning, evaluating and summarizing progress)● Assisting with creating natural supports and accessing community resources that support recovery● Provide back-up services for peer team members● Actively support client self-reliance● Collaborate with other staff members, community agencies/supports, and family members about clients mutually served● Complete required documentation in a timely manner including strengths assessments; personal treatment plan; safety plan; and progress notes● Demonstrate awareness and sensitivity in the delivery of client services as it relates to the cultural and socioeconomic characteristics of the populations served● Collect information regarding clients’ history through a combination of interviewing, records reviews, and consultation, after obtaining written consent
REQUIREMENTS
3 years of related experience;in behavioral health
Bachelor’s degree in social work, Psychology or other human services field and/or a minimum of at least 3 years’ experience and able to qualify as a QMHA under MHACBO, within 60 days of hire substantial or relevant experience will be considered in place of degree
QMHA certification is required upon start / within 60 days of hire. Credentials may be required depending on individual program or location requirements.
Fluency in English, both spoken and written
Intermediate mastery of business applications and productivity suites

Travels between worksites regularly; responsible for transporting clients in personal and/or agency vehicle, which requires valid driver’s license, auto insurance, and motor vehicle records clearance
Works in a variety of settings, including and not limited to LCSNW office locations, partner agency and community-based settings, client home/community settings, outdoor settings, and faith-based settings such as churches
Meets LCSNW criminal background clearance requirements and other job-relevant clearances as required by the State or program

Salary : $21 - $31
