You are a compassionate and skilled social services professional dedicated to improving the quality of life for elderly residents. You possess strong interpersonal, verbal, and written communication skills, and are adept at building relationships with residents, families, and staff. Your ability to organize, prioritize, and manage multiple tasks and responsibilities is exceptional. You have a comprehensive understanding of the long-term care regulatory environment and are committed to maintaining compliance with state and federal regulations. Your leadership skills enable you to effectively manage and inspire your team to achieve the highest standards of care and service.
What You’ll Do
· Oversee quality assurance and improvement for the social services department.
· Develop and implement policies, standards, and objectives for the department.
· Identify and lead new programs to support residents and families.
· Ensure compliance with regulatory agencies, including ISDH and Medicaid/Medicare requirements.
· Monitor and respond to all pre-screening and related state/federal paperwork.
· Serve as the facilitator for Resident Council Meetings and address resident concerns.
· Conduct initial and routine assessments with residents and motivate their participation in activities.
· Serve as designated staff for resolving customer concerns and documenting follow-up actions.
· Coordinate residents' relocation and discharge planning.
· Assist residents in acquiring outside resources and benefits.
· Manage and evaluate social worker performance, providing feedback and professional growth opportunities.
· Participate in marketing and admissions activities, providing tours and counseling families.
· Engage in organizational leadership and quality assurance meetings.
· Assist in budget planning and regulatory surveys.
What You Need
· Bachelor’s degree in social work or a related field.
· Two years of experience in geriatric social work or services to the elderly.
· Strong interpersonal, verbal, written, and assessment skills.
· Ability to organize and prioritize work to meet deadlines.
· Comprehensive knowledge of long-term care regulations related to social services and admissions.
· Supervisory experience in social work preferred.
