What are the responsibilities and job description for the Clinical Liaison position at Lutheran Senior Services?
Summary:
This position is responsible for educating referral sources, obtaining referrals, conducting clinical screens, and participating in planning, organizing, and executing the LSS marketing plan for post-acute services. The individual will be responsible for establishing and maintaining effective working relationships with their assigned accounts as well as those of their customers and LSS team members. Daily travel within assigned territory is required. This position upholds the Lutheran Senior Services philosophy, core values, and goals to ensure that the Christian mission of “Older Adults Living Life to the Fullest” is realized.
Responsibilities
Establishes and maintains effective working relationships with referral sources, LSS communities, and key stakeholders
Effectively markets to assigned accounts to generate appropriate referrals and admissions
Tracks available inventory (i.e. bed availability)
Meets budgeted and strategic goals
Promotes LSS as the post-acute provider of choice in the hospitals for patients, physicians, and employees
Stays current on and applies a firm understanding of Medicare and Medicaid regulations related to the various programs and services
Stays current on and applies a firm understanding of current insurance contracts across the LSS continuum
Meets with patients and families at hospitals to coordinate LSS post-acute services
Visits all LSS residents who are hospitalized to ensure they are returned to an LSS community with LSS services whenever possible, coordinating services with the hospital discharge planners
Performs accurate and thorough initial on-site screening of potential patients
Completes screening within established timeframes and determines clinical acceptance based upon LSS criteria
Connects referrals to the entire LSS continuum—REACH, CCRC, or In Home Services
Works with designated LSS employees to complete the admission transaction into the appropriate level of care or service
Is in daily contact with hospital referral sources and LSS communities in the assigned territory either in person or via telephone or email
Conducts one-on-one calls within assigned territory per week and completes a detailed call plan for each call
Coordinates external marketing efforts with Health Services Counselors and Marketing Representatives for In-Home Services with support from the Regional Director of Sales for Health Services
Establishes a positive image of LSS in the community
Maintains a high standard of excellence by promoting a positive, productive, and compassionate customer experience, always striving to exceed customer expectations and treating patients, residents, guests, clients, families, physicians, visitors, vendors, and coworkers with dignity and respect
Promotes an atmosphere of teamwork, emphasizing a group effort in which the contribution of every employee is essential
Participates in weekly calls with team
Participates in on-call rotation with other clinical liaisons
Tracks appropriate information in the database throughout the sales process daily
Meets with the Director of Regional Sales for Health Services to develop and review quarterly goals and action plans
Completes quarterly key account profile
Qualifications, Knowledge, Skills & Abilities

Bachelor’s degree from an accredited college or university with emphasis in nursing, social work or business/marketing; and/or commensurate experience in marketing/sales of health services
Nursing degree preferred, but not required in the presence of related hospital experience
Knowledge of the post-acute industry including admission criteria
Knowledge of Medicare, Medicaid, and Managed Care
Ability to travel daily within assigned territory
Excellent speaking, writing, and sales skills
Strong customer service skills
Self-motivation to work independently and as a team member
Advanced knowledge of computers and the Internet
Database experience
Demonstrated skills and ability to communicate effectively in both verbal and written forms with medical administrative staffs as well as patients and families at providers and LSS communities in an assertive, professional manner.
Self-motivation, self-direction, and excellent organizational skills, with the ability to effectively handle multiple tasks simultaneously.
Ability to compute statistics and complete necessary reports in a timely and accurate manner
Ability to complete the clinical screens accurately and thoroughly within established timeframes with demonstrated clinical expertise to determine admission criteria
Proficiency in the use of technology to complete tasks and maximize efficiencies
