Establishes a network with community-based service providers to facilitate resident’s access to programs. These may include, but not limited to, formal assessment, personal assistance, homemaker services, meal services, transportation, counseling, occasional services from a visiting nurse2. Provides general case management and referral services to all residents needing such assistance by educating residents on service availability, applicant procedures and client rights3. Monitors the ongoing provision of services from community agencies and keeps the provider agencies informed of progress of residents4. May hold the responsibility of coordinating their site’s volunteer program in conjunction with a designated member of the LSS Volunteer and Community Engagement team, which may include determining volunteer needs, onboarding and orientation, record-keeping, evaluation, and retention of the volunteers5. Works with residents and community service providers to enhance services by implementing new on-site services and programming, such as preventative health screenings6. Participates in resident councils and other resident groups to improve communication between residents and management7. Assists residents applying for entitlement programs such as Medicaid, SSI, and food stamps for which they may be eligible; Coordinates and oversees the delivery of supportive services and provides information on available services8. Assists in the development of on-going, wellness-oriented, on-site programming for residentsQualifications, Knowledge, Skills & AbilitiesTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required.•Bachelor of Social Work or degree in Sociology, Gerontology, or Psychology or comparable experience•Two years of prior experience in social service delivery with senior citizens or non-elderly persons with disabilities•Demonstrated working knowledge of supportive services and other resources for senior citizens and non-elderly persons with disabilities in the jurisdiction of the project•Ability to advocate, organize, problem solve and provide results for the older adults and non-elderly persons with disabilities•Ability to relate well to family members and other constituencies•Ability to develop cooperative relationships with peers and supervisors•Ability to manage time and set priorities•Computer literacyPhysical Requirements and Working Conditions:The physical activities and demands described here are representative of those an employee encounters while performing the essential functions of this job.
