As a Care Coordinator at Lutheran SeniorLife, you will be part of an interdisciplinary team responsible for identifying and developing care plans that address the psycho-social needs of residents/patients. Your role will involve ensuring appropriate care plans, advocating for residents/patients, staff education, and developing goals for residents/patients to achieve.
Qualifications
- A current license under the Social Workers, Marriage and Family Therapists, and Professional Counselors Act or a bachelor's, master's, or doctoral degree from an accredited school or program in social work or social welfare.
- One year of experience in a health-related field.
- Strong written and verbal communication skills.
- Working knowledge of computers, Microsoft Word, and Excel.
