- Utilize evidence-based practices during individual and group treatment that supports client’s goals and addressed mental health needs/symptoms.
- Assess patient throughout their episode of care to determine medical necessity and create an individualized plan of care for the identified diagnosis.
- Communicate with supervisor, treatment team and collateral agencies in order to develop and execute treatment plans.
- Provide and coordinate a continuum of treatment services between the EO/SCH and BHAL referrals to meet the client’s needs.
- Comply with productivity standards.
- Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, other LSSI workforce members, and collaborative organizations and/or individual in a dignified, respectful, honest and fair manner.
- Master’s degree in social work, counseling or similar field required.
- LCSW/LCPC required.
- CADC preferred.
- Minimum of 1 year experience working with adult clients with a mental health diagnosis, conducting counseling, crisis intervention or case management required.
- Bilingual, written and verbal preferred.
- Background check clearance required.
- Must pass the IL IM-CANS training/test.
- Demonstrated ability to communicate in a clear, comprehensible manner, both verbally and in writing.
- Excellent communication, organization, presentation and pc/computer skills (including proficiency with Microsoft Office – Outlook, Word, Excel and PowerPoint) along with other related software.
