The position is part of the Enhanced Care Management Team which provides community based, high-touch case management services to individuals who are high utilizers of hospital services. Case management services assist in achieving and maintaining stability as well as self-improvement and self-sufficiency.
Required Skills and Experience
- BA or BS degree from an accredited college or university, preferably with a major in a behavioral science, and 4 years experience in the human services field.
- Maintain a strength-based perspective.
- Able to work with a variety of community organizations and health providers.
- Treat others with respect and courtesy, striving for open and honest working relationships.
- Ability to work with adults or youth with low income from diverse social and ethnic backgrounds who have a history of homelessness.
- Demonstrated knowledge of Trauma Informed treatment framework.
- Professional experience working with clients who have AOD dependencies, mental health diagnoses, domestic violence histories, issues with abandonment, and abuse.
- Ability to access community-based services and to collaborate with other service providers.
- Has a clean driving record, licensed and registered car, and proof of insurance.
