Schedule:
Monday – Friday (9am-5pm)
Principal Responsibilities:
Manage a caseload of assigned families requiring housing stabilization and supportive services following placement from family shelter.
Provide case management services including assessment, budgeting, employment search, arranging childcare, referrals to other community supports, and creation and monitoring of housing plan. This should include discussions about career interests, benefit assistance, strengths/barriers, mental/physical health status, legal issues, financial obligations, and current support systems.
Collaborate with clients in the formulation and maintenance of a dynamic housing stability plan that is reviewed and modified at least every two weeks
Schedule and attend in-home visits and/or remote visits with families a minimum of once per month for basic Stabilization and 2 times per month for Intensive Stabilization.
Document case management services through detailed and thorough case notes and updates to stabilization plans in accordance with LSA case note policy.
Maintain organized client files both hard copy and electronic in compliance with contract and/or funder requirements. Files will include but are not limited to; eligibility/intake paperwork, assessments, case notes, stabilization plans, income/benefits, and other required documents.
Develop a network of social service providers, and refer participants with appropriate services such as financial assistance, entitlements, addiction services, advocacy, employment assistance, financial counseling, rental assistance, deposits, Domestic Violence services, Mental Health services, etc. and assist with follow up for those referrals.
Assess needs such as food, furnishings, kitchen supplies, clothing, school supplies, etc. and initiate referrals for identified needs.
Act as liaison between families and landlord for compliance with lease, eviction prevention, good neighbor behavior, major maintenance issues, etc.
Apply knowledge of residential lease contracts and landlord laws to educate clients of their rights and responsibilities

Qualifications:
*Experience working with people from very diverse backgrounds
*Excellent communication and conflict resolution skills
*Ability to implement and utilize limit setting techniques and appropriate boundaries
*Ability to speak, read and write English, bi-lingual preferred
*Ability to pass CORI/background check
*Posses a valid Massachusetts driver's license and reliable transportation
