Overview
Join our M Health Fairview Rehabilitation team at Bethesda LTACH, located in the Fairview Community Health and Wellness Hub! We are seeking a casual physical therapist to be part of a robust multidisciplinary therapy team. In collaboration with patients and families, the physical therapist is responsible for providing safe and effective delivery of patient care within scope of practice. This includes examination, evaluation, diagnosis, planning, intervention and establishing outcomes.
- FTE 0.01, Casual
- Schedule: One weekend shift per month, weekday shifts available
- Eligible to work up to 80 hours per pay period, pending shift availability

M Health Fairview Rehabilitation offers a broad range of services that serve patients across 10 acute-care hospitals, 3 post-acute settings and 70 outpatient adult and pediatric therapy clinics. Consisting of Physical, Occupational and Speech Therapy as well as Audiology and Cardiac & Pulmonary Rehab, our therapists collaborate with colleagues in all medical settings and offer dozens of specialty programs.

Responsibilities/Job Description
The Physical Therapist partners with the patient and family while performing evaluation and re-evaluation. Captures and applies pertinent data, performs examination, and resolves treatment diagnosis.
Develops an individualized plan of care through coordination
Analyzes data to identify needs and potential risks, settles prognosis for recovery, and establishes individualized goals
Implements and modifies care plan based on reassessment and patient response
Coordinates care and communicates effectively with interdisciplinary team for all aspects care & establishes appropriate discharge plan
Partners with patients and families to provide skilled intervention
Implements and alters treatment to reflect change in patient status and response to treatment
Assesses patient and/or caregiver learning needs and provides appropriate education
Documentation/Billing
Uses acceptable medical terminology and follows professional practice standards.
Completes documentation in a timely manner per department standards
Stays in sync with department billing standards
Professional Expectations
Prioritizes tasks and patient care for effective time management.
Understands and follows professional licensure/certification requirements
Advises team members and/or supervises students as designated
Pursues professional development to support clinical standard processes
Quality Improvement
Crafting and implementing improvements, and participates in and supports entity/system initiatives
Meet or exceeds department standards for productivity
Qualifications
Required
Physical Therapy registration/licensure in the state of Minnesota or temporary license in MNCurrent Basic Life Support (BLS) certification
Preferred
Doctorate degree in Physical Therapy1 year of physical therapy experience
