Working in a community residential program for 4 men with acquired brain injuries as an important part of their residential services team.
- Provide supports to participants with disabilities in residential settings including wound care, medication administration, g-tube care, health care coordination, and advocacy
- Assists with the development and implementation of health care standards, policies and procedures.
- Provides training and consultation to direct care staff regarding policies and procedures according to Medication Administration Program (MAP) policy.
- Reviews all incident reports involving health and safety issues, including mental health and behavioral incidents.
- Oversees MAP Program at the residence by conducting routine audits of documentation system.
- Conducts reviews of health care documentation to ensure program compliance with the provider’s recommendations as well as MAB, DDS policies and the MAP policies.
- Identifies and facilitates the utilization of appropriate community resources.
- Transports participants to medical appointments and provides advocacy at the appointment.
- Administers medications and treatments as prescribed.
- Works as a part of the wider LPN/RN nursing team.
CERTIFICATES, LICENSES, REGISTRATIONS
- Current Licensed Practical Nurse (LPN) License
- Current, valid, US drivers' license
- All candidates must be able to successfully pass a CORI, reference, multi-state background and driving record check.
