GENERAL SUMMARY
Provides coordination of services for clients by conducting intake assessments and interviewing clients for program eligibility. Assists with the following programs: emergency services, asset building, tax preparation, money management and financial literacy, weatherization assistance, minor home repair, senior chores services, home injury prevention, transportation, food programs, senior programs, Michigan Medicare/Medicaid Assistance Program, housing stability and homelessness assistance programs and Water Residential Assistance Program. Performs related duties as assigned.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
Processes applications for assistance and verify documents necessary for multiple programs.
Provides clients with case management and follow up to ensure self-sufficiency.
Reviews and evaluates client circumstances and documentation.
Advocates on behalf of low-income individuals and families, to community organizations, service providers and institutions.
Corresponds with apartment complexes, management companies, private property owners, mortgage companies, utility companies, Department of Health and Human Services, other community partner agencies, and municipalities.
Ensures compliance with the requirements of the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and maintains confidentiality of protected health information at all times.
May conduct community outreach for programs and participate in meetings as assigned.
Maintains accurate electronic and/or paper files by processing application, keeps case history files current and responds to client inquiries and complaints.
Answers and records telephone information pertaining to crisis interventions, pre-screening of customer information, customer follow up and the completion of forms.
Maintains knowledge of organizations that provide community services; seeks referral information using directories and resource guides.
Operates automobile while performing assigned duties.
QUALIFICATIONS
Required Education and Experience
  • Associate’s degree from an accredited college or university in Human Services, Social Work, or Business
Required Licenses or Certifications
  • Valid Michigan driver’s license and an operable insured automobile for authorized travel.
Preferred Education and Experience
  • Bachelor’s degree from an accredited college or university in Human Services, Social Work, or Business
  • Previous human services work experience
