Vision & Overall Purpose of Position
The Support Coordinator is responsible for managing a multifaceted caseload, functions as a lead person in coordinating and assisting families in developing specialized services for those with an intellectual or developmental disability. The caseload requires the utilization of and collaboration with community agencies serving those with an intellectual or developmental disability. The Support Coordinator acts as a lead member of a multidisciplinary treatment team in determining the proper level of support/service to purchase or procure for individuals, and may involve responsibility for conducting specialized client assessments.
Reports To: Support Coordinator Supervisor
Classification: Non-Exempt, Full Time
Qualifications:
- Bachelor’s degree or Registered Nurse from an accredited college
- Must have current valid driver license and be willing to obtain a Class E license immediately upon hire
- Must possess a reliable vehicle
- Ability to pass all appropriate/ required background screenings
- Effective written and verbal communication skills
- Proficient use of Google docs and database systems

Essential Functions of the Position:
- Displays and demonstrates knowledge of Targeted Case Management practices and policies and is able to communicate these to others
- Develops, in cooperation with the individual, family, and provider agencies, an appropriate person centered plan for each client
- Monitors services by vendors to ensure that the terms of the authorization are being fulfilled, services are being delivered in a quality manner, and to review progress being made
- Represents the agency at local, state, and national conferences / professional organizations
- Knowledge of and display competency in DMH rules and directives and Medicaid guidelines
- Knowledge of and display competency in legal and regulatory guidelines that apply to services and resources
- Reviews all monthly reports relating to the individual and acts accordingly to such reports
- Maintains accurate documentation and ensures documentation is completed within the required timelines
- Maintains confidentiality of client information
- Promotes self determination through informed choice
- Promotes competitive integrated employment services
