Overview
Coordinates services to adequately meet client’s needs, responsible for assessing the needs specific to the individual case, developing an appropriate comprehensive care plan, and arranging for services and follow-up as needed. Work is performed under the direct supervision of the Program Coordinator/Deputy Director or OFA Executive Director.
Responsibilities
- Make home visits, assess client needs, design appropriate individual care plan/service plan, arrange and coordinate services for clients;
- Follow-up and monitoring of the provision of services to clients to ensure needs are met as directed by NYS Office for the Aging
- Speak to service organizations;
- Prepares a variety of records and reports related to work;
- Assist with workshops, health fairs, Caregiver training and support groups;
- Keeps abreast of current trends in long term care, attends related conference, seminars, etc.;
- Works with providers, health aides, human and social services agencies to identify and assist with applying for types of services available, and to ensure timely services;
- Monitors and maintains an ongoing tracking process, to ensure that program activities and expenditures are implemented according to plan;
- Assume any other duties which may be assigned.
Required Knowledge, Skills, and Abilities
Good knowledge of the characteristics, needs and interests of the aging; good knowledge of community agencies, facilities and services which can be utilized to aid the elderly; ability to make accurate assessment of social, emotional, physical and financial needs of elderly clients using prescribed procedures; ability to communicate clearly and effectively both verbally and in writing; ability to organize; initiative and resourceful; tact; courtesy; integrity; physical condition commensurate with the demands of the position
Acceptable Experience/Training:
(A) Graduation from a regionally accredited or New York State registered four year college or university with an Bachelor’s Degree in Human Services or related field; or
(B) Graduation from a regionally accredited or New York State registered two year college or university with an Associate Degree in Human Services or related field and two years of experience in community organization or the field of aging; or
(C) High School education or equivalent and four years’ full-time experience in a community organization or the field of aging doing social casework; or serving as a case aide.

Pay: $20.19 per hour
