What are the responsibilities and job description for the Case Manager position at Madison Haywood Developmental Services?
BRIEF JOB SUMMARY: The Case Manager assists the Director of Nursing with the overall compliance of the agencies practices in regards to regulations set forth by the Department of Intellectual and Developmental Disabilities; Licensure; Department of Health; and TNCare.
ESSENTIAL JOB FUNCTIONS:
- Enter/and or reconcile medications onto the EMAR as needed as a backup person.
- Review and work up all required paperwork consults associated with medical appointments in a timely manner, input appointments in TIMAS. Communicate with DON to ensure proper medical treatment was implemented.
- Make phone calls to the homes of PS that receive orders for new medication to verify that meds were received. Destroy medication that has been discontinued.
- Assist with medical issues that may arise.
- Notify home staff of upcoming appointments at least 24 working hours before scheduled appointments via phone giving details of appointment and making sure they have needed paperwork; follow up next day with home staff to verify if appointment was completed and outcome.
- Assist with gathering information for audits and surveys
- Assist with all vaccine clinics that are held, gathering PS information such as insurance cards etc.
- Conduct daily hospital checks on all PS that have been hospitalized and manage grid for admission date, discharge date and follow up
- Oversee and manage psych clinic by obtaining list and sending out to the appropriate individuals, sending iMind medication list of PS that is scheduled prior to appointment and ensuring all lab work is completed and sent to iMind as ordered.
- Maintain a professional attitude with Persons Supported and their family, co-workers, subordinates, therapists, practitioners, ISC’s and all other agency stakeholders by presenting in a courteous, respectful manner at all times.
QUALIFICATIONS:
- Good organizations skills
- Case management or medical experience is a plus
- Must be a team player
- Valid Driver’s License.
- TB Risk Assessment Questionnaire and free of illegal substance.
- Clear background of felonies and abuse offenses.
- Must maintain medication certification

Pay: $17.00 per hour
