Essential Functions and Responsibilities :
Assisting residents with ADL tasks as indicated in their service plans. This includes but not limited to : assisting with bathing, grooming, toileting, dressing, oral care, ambulation, transfers, serving and assisting with meals, feeding, housekeeping, bed making, and laundry
Always observe and uphold resident rights
Positively supports residents with emotional and behavioral issues
Assist other staff as needed on the floors
Assist residents to and from meals
Supports resident activity participation by assisting residents to / from activities, encouraging participation, and leading activities when asked.
Must be willing to work with staff from all departments to ensure quality of care for residents
Keep environment safe and clean for residents; performing housekeeping duties as indicate don cleaning schedule or assignment sheets.
Observe residents for safety and report any and all changes in resident condition effectively, accurately, and in a timely manner to LN on duty
Assist in development and revision of Service Plans by informing supervisor of current resident needs and preferences as well as successful service approaches.
Answers all resident pendant / pull cords immediately.
Carry and possess a working knowledge of community call / communication system
All other duties as assigned.

Must be neat in appearance, pleasant personality, be congenial to fellow team members, residents, families, and the public in general. Must be capable of using own judgement and initiative in daily duties.
Always demonstrates safe working practices.
Maintains a professional image to residents and fellow co-workers in words, actions and in appearance
Always maintains confidentiality and privacy of our residents and staff.
Relate to residents in a courteous, empathetic, and cooperative manner.
Must be a mature individual. Must possess the ability to multi-task in a fast-paced environment and perform a variety of tasks, often changing assignments on short notice.
Must have strong organizational skills, as well as the ability to prioritize and meet deadlines.
Must be detail and team-work oriented.
Must attend required in-services / workshops to further education of skills and community requirements.
Qualifications :
Must be Drug free and submit to Urinary Analysis Drug screening upon hire and routinely.
Must be able to pass Criminal Background Check, Health Screening including TB test.
Must be eighteen (18) years old.
Maintains current CPR and first aid certification.
Ability to read, analyze, and interpret care plans.
Assisted Living and Memory Care regulations knowledge.
Ability to prioritize and organize assignments.
The employee shall be responsible for maintaining the integrity of Fields Senior Living
Physical Demands and Requirements :
Maintain ability to lift 50 pounds; to stand or walk throughout the vast majority of the work day; and to frequently push, pull, bend, twist and reach.
Must be willing to be active and on feet during entire work shift.
Involves light to heavy physical activity performing non-strenuous and strenuous activities.
Must have sufficient manual dexterity to work with the fingers and to reach / hand items.
