Position: Behavioral Health Services Case Manager (Lewiston/Auburn)

Pay rate: $25-$27 per hour based on experience
Location: Main Street Office, Lewiston ME
Job status: Full Time (8:00-5:00pm) Some evenings or weekends based on client availability.
Job Summary:
The Behavioral Health Services (BHS) Case Manager works with eligible families with moderate to minimal case management requirements to assist them in identifying their strengths and needs. The Case Manager works with the family to develop an individualized service plan that includes appropriate services and supports that meet the needs of the child and their family. The Case Manager informs the family of services and supports and assists, when needed, in coordinating services and supports.
Primary Responsibilities:
The primary responsibilities of this position include, but are not limited to, the following:
- Screen and enroll eligible participants in the BHS Case Management Program and provide linguistically and culturally appropriate service coordination; including assessing client’s needs.
- Conduct intake services with clients to assess needs and complete all necessary intake paperwork to maintain compliance and conduct a comprehensive needs assessment with participants.
- Facilitates in developing an Individual Support Plan, 90 days review and annual plan
- Assess the client’s strengths and needs and develop a holistic individualized family plan for each client and
- Provide wrap-around linguistically and culturally appropriate case management for clients.
- Assist clients and their families in applying to external resources.
- Conduct home visits to client homes as necessary.
- Develop self-sufficiency plans and timelines in partnership with each client.
- Provide individualized support through direct services, referrals, and advocacy.
- Provide cross-cultural information, orient individuals and families to their community.
- Assess and monitor client progress to support their attainment of established goals, address challenges, and to ensure that available resources are utilized.
- Understand and adhere to the guidelines and requirements of BHS case management services
- Maintains a minimum caseload of clients required by the agency.
- Must perform billable hours within the first 60 days of employment.
- Must maintain 25 hours billable per week
- Renegotiation of salary will be triggered if below 25 hour/week minimum for 4 weeks in a row or for 2 weeks a month for 3 consecutive months
- Informs children and families of the rights of parents as recipients of mental health services.
- Coordinates services for the client evaluates client progress and satisfaction with services and monitors the effectiveness of services in meeting the needs of the client.
- Advocates for the protection of client rights and entitlements as outlined in the Rights of Recipients of Mental Health Services
- Maintains ongoing relationships through home visits, phone contacts and Collaboration with clients, families and service providers,
- Strives to empower families with the skills to navigate the mental health system.
- Complies with all agency case management standards.
- Must be willing to work with clients with diverse cultures.
- Maintains complete records for all clients on caseload.
- Completes any required documentation in a timely manner (i.e. client record information, evaluation data, progress notes, request for funds, etc.).
- Complete and maintain all BHS CM client files to ensure program documentation compliance.
- Ensure that all formal client interactions are documented and retained thoroughly, accurately, and expeditiously in the client management database.
- Detail each client interaction in accordance with program requirements.
- Input client information into MEIRS database(s) and assist with tracking and reporting as needed.
- Review past BHS CM client files and complete necessary documentation with clients to ensure client and program file compliance per funding/program requirements.
- Scan completed documents to MEIRS server.
- Collaborate effectively with coworkers and partner organizations.
- Build and maintain relationships with area service providers for the benefit of clients.
- Work to connect clients with supportive community networks and support systems to ensure their long-term integration in the community.
- Attend stakeholder meetings and work with community partners to learn about resources for refugees and make referrals when needed.
- Use personal, insured vehicle and/or public transportation (where available) to travel and transport clients and materials as needed throughout the service delivery area.
- Maintain a clean driving record and personal vehicle auto insurance with at least $100,000/ $300,000 liability limits to transport clients as per MEIRS transportation policy.
- Adopt, implement and promote agency values and philosophy in the course of employment with the agency.
- Maintain all documentation and reporting requirements for this position.
- Attend all staff and program meetings.
- Submit weekly and monthly reports to direct supervisor
- May train and lead the activities of volunteers and interns.

- Arrange for interpreter services, if necessary.
- Prepare narrative and statistical reports and submit to supervisor
- Enter data into PIMSY/other database MEIRS uses in recording all client services in a timely and accurate fashion.
- Complete and maintain physical file documentation in case files in a timely and confidential manner.
- Attend and participate in staff meetings, supervisory meetings, other meetings, and training programs.
- Assist in the coordination of activities with other MIERS programs and work as a member of a team.
- Staff is expected to be professional including but not limited to; dress code, communications with peer and other clients,
- Must keep confidential of staff, client and agency information
- Submit timely: timesheets, mileage and personal expenses and other required administrative tasks
Minimum Qualifications and Skills Requirements:
- Bachelor’s Degree required and Master’s Degree preferred.
- Minimum Two (2) years Case Management Experience or similar field, required.
- Experience with both children and adults is preferred.
- Have a knowledge base of an Electronic Medical Records (EMR) system preferred
- MHRT/C certification
- Self-directed, motivated and able to handle multiple projects simultaneously.
- Knowledge of the public benefits system in Maine, e.g., MaineCare, SSI, WIC and subsidized housing programs.
- Strong written communication skills, ability to write reports and clear case notes.
- Detail oriented, extensive documentation skills, organized and able to work in a multi- tasked environment.
- Skilled at crisis management, problem solving, decision making and mediation.
- Cross-cultural sensitivity and knowledge with the ability and desire to work with people of other cultures.
- Must work well in a team setting.
- Have a valid driver’s license and access to own transportation
- Required computer skills: Word, Excel, Internet, Gmail, Outlook. Comfortable in utilizing new programs and databases.
- Multilingual skills preferred
- Must be able to pass background checks and maintain a clean DMV record
