Pay: $20 - $24 / hour depending on experience
This is a hybrid position with two remote days and three office days. The office is located at 340 County Rd, Westbrook, Maine. The schedule is 8-4:30, Monday-Friday.
The Health Coach is a trusted care team member who provides wellness coaching and support to patients with Hypertension. In collaboration with the Advanced Practice Provider, Pharmacist, or designee, the Health Coach will partner with patients to support their Hypertension diagnosis in a remote patient monitoring program. This position will involve onboarding patients into the program over the phone and maintaining care through scheduled telephone or video contact. The Health Coach will establish concrete goals to improve patient health and wellness.
Required Minimum Knowledge, Skills, and Abilities (KSAs)
- Education: High School Diploma preferred. Bachelor’s Degree preferred.
- License/Certifications: Valid Maine Driver’s License. Health Coach certification preferred.
- Experience: 2 years' working in a social service or health care setting or equivalent educational setting preferred.
- Demonstrated ability to work with individuals and families who have complex social and clinical needs preferred.
- Bilingual preferred.
- Sensitivity to cultural and ethnic diversity preferred.
- Local community resident with good knowledge of the resources available in the community preferred.
- Excellent organizational and critical thinking skills and ability to prioritize workload and to demonstrate flexible problem solving approach.
- Excellent communication and interpersonal skills and abilities to respond to diverse needs from patients, families and employees.
- Familiarity with medical terminology preferred. Ability to project a pleasant and professional image even under stressful situations. Ability to maintain confidentiality.
- Must complete most duties with minimum direct supervision and demonstrate ability to collaborate with teams of individuals in diverse settings.
- Ability to travel frequently to PCP practices and other sites where patients receive care. Proficiency in computer use, internet and health information technology; knowledge of EPIC electronic medical record system preferred.
