What are the responsibilities and job description for the Social Worker position at Manahawkin Health and Rehabilitation Center?
Manahawkin Health and Rehabilitation Center is hiring for a passionate Social Worker with healthcare experience to join our social services team in Manahawkin, NJ. As a Social Worker, you will work closely with our residents to ensure quality care is provided to improve the lives of our residents.
This is a full time position working Monday- Friday day time hours.
Duties and Responsibilities :
- Maintaining and / or improving resident’s ability to control everyday physical needs and mental / psychosocial needs
- Identifying individual social and emotional needs
- Developing and maintaining individualized care plans
- Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
- Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
- Work with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stay
- Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community
