POSITION SUMMARY

The Behavioral Health Case Manager will provide community-based support services to eligible recipients to establish, organize, integrate, and modify the treatment and social support services and resources required to meet recipient needs. Services are to be provided in collaboration with the recipient for the following purposes: (A) Facilitate provision of community-based social, educational, vocational, legal, and financial resources; (B) obtain medical, behavioral health, or other specialized treatment services and social support services; (C) monitor the safety and stability of the recipient and connect the recipient with crisis intervention services; (D) coordinate the provision of services and exchange of information between treatment providers and social services agencies to help achieve the goals listed in the recipient’s treatment plan; (E) help the recipient engage in social relationships and natural community-based supports that enhance the recipient's quality of life; and (F) help the recipient resolve conflicts and mediate solutions to problems with treatment providers, social support agencies, or natural community-based supports. This position reports directly to the Counseling and Recovery Supervisor and/or designee.


PRINCIPLE DUTIES AND RESPONSIBILITIES

1. Case Manager serves as the central point of contact for an individual, brokering and/or linking individual with mental health, SUD, medical, social, educational, vocational, legal, and financial resources in the community.

2. Intensive outreach services outside of clinic, including street outreach, visiting the client’s home, work, and other community settings.

3. Referring for individual, group or family therapy, medical, or other specialized services.

4. Engaging natural supports (family members/close kinship relationships and community members (e.g. friends, co-workers, etc.)) that enhance the client’s quality of life.

5. Support primary clinicians with assessments and treatment plans, including quarterly updates.

6. Regular (biweekly, at a minimum) monitoring of behavioral health services, delivery, safety, and stability.

7. Triaging for crisis intervention purposes (e.g., determining need for intervention and referral to appropriate service or authority).

8. Assist client in being able to better perform activities of daily living, such as problem-solving skills, self-sufficiency, productive behaviors, or conflict resolution.

9. Assists client in obtaining appropriate releases in order to ensure quality of care across treatment teams and departments.

10. Prepares and completes all required documentation in a timely manner.

11. Appropriately recognizes and maintains the confidentiality of all patient information.

12. Maintains confidentiality of all client, staff, and division-related information as per State and Federal regulations, Maniilaq Policies, BHS policies and procedures, and Staff Code of Conduct.

13. Provides coordination of care services, ensuring that client receives necessary medical, mental health, and social support as a part of their therapy or treatment.

14. Assists client in accessing and processes for Medicaid and other third-party billing for all therapy and treatment services.

15. Directly participates in discharge and transitional planning.

16. Prepare monthly and quarterly reports as required by grants and contracts.

17. Promotes good public relations with the community, regional organizations, State and Federal agencies, and Manilliaq Counseling & Recovery (CRC) personnel.

18. Work closely with CRC itinerant therapists, clinical lead, and clinical supervisor to ensure programs are in compliance and appropriate service delivery.

19. Ability to travel on short notice and in small planes required.

20. Must be able to navigate office equipment such as computers, printers, fax machines and copy machines.

21. Must also be willing to learn multiple computer programs and take on multiple duties and responsibilities.

Job Qualifications:


MINIMUM REQUIREMENTS

Case Manager I, Range 13:

High school diploma or equivalent. Administrative experience preferred. Must possess current Alaska driver’s license and have a good enough driving history to be insured to drive Maniilaq vehicles, or obtain within a year of hire. Possesses good communication skills and an aptitude for working with people. Proficient verbal communication and writing skills.


Case Manager II, Range 14:

Has met all the skills and requirements of BHCM I. Must have learned, practiced, and been evaluated for basic service delivery skills through direct supervision with clinical supervisor or designee for at least 50 hours (no more than 10 hours may be group supervision). Must be considered proficient by direct supervisor. Must have a total of 1500 hours of general work experience including a minimum of 500 direct client care hours. Must also be willing to perform duties from BHCM I if needed.


Case Manager III, Range 15:

Has met all the skills and requirements of BHCM II and must have learned, practiced, and been evaluated for basic service delivery ability through direct supervision with clinical supervisor or designee for at least 50 additional hours (no more than 25 hours may be group supervision). Must be considered proficient by direct supervisor. Must have a minimum of 1000 additional hours of general work experience including a minimum of 500 direct client care hours.
