POSITION SUMMARY
The Integrated Care Counselor will be responsible for delivering brief interventions and referrals in Outpatient Services, Inpatient Services, Women’s Health, telemedicine, and emergency room settings. This is a dynamic role that involves addressing a wide range of social, behavioral health, and substance abuse treatment needs in a rural/remote Alaska Native population. The Integrated Care counselor provides same day and brief limited services to patients in the medical setting to facilitate low barrier access to behavioral health services. The Integrated Care Counselor reports directly to the Integrated Care Lead Therapist.
PRINCIPAL DUTIES AND RESPONSIBILITIES
1. Performs behavioral health, substance use, and social needs assessments in primary care and/or emergency room settings.
2. Performs screenings, brief interventions and referrals to treatment (SBIRTS).
3. Performs biopsychosocial assessments to handoff clients into community behavioral health care services.
4. Delivers interventions to address behavioral health and substance abuse needs.
5. Applies behavioral principles to address modifiable lifestyle and health risk issues.
6. Connects patients to social and tribal services to address relevant social needs.
7. Maintains awareness and promotes coordination of social, tribal, health, legal, community behavioral health, and substance abuse treatment services in order to provide comprehensive and holistic patient care.
8. Communicates effectively with the care team, including providers, nursing staff, patient advocates, CHA/Ps, behavioral health aides, and case managers.
9. Member of the behavioral health team as part of the crisis on-call rotation.
10. Attends all meetings, trainings, and case staffings as directed by supervisor.
11. Documents treatment process and progress in EHR to meet all relevant requirements and to support well-coordinated patient care.
12. Works as part of a team to provide optimal behavioral health and substance abuse treatment services, which may include making referrals to other Maniilaq and non-Maniilaq programs.
13. Maintains confidentiality of patient records and treatment.
MINIMUM REQUIREMENTS
Integrated Care Counselor I:
Must have a Master’s degree in Social Work, Professional Counseling, Psychology, or Family and Marital Therapy (or the like). Master’s degree must be an eligible Master’s for an Alaska State licensure at the time of hire. If the candidate does not have a license eligible Master’s degree but has obtained a Doctoral degree that is eligible for an Alaska State Licensure, the Doctoral degree will be accepted as equivalent. Must be able to obtain an LCSW, LPC, LMFT, or LAP within 2 years. Broad clinical experience. Ability to work with sensitivity in a cross-cultural environment. Ability to adjust to “bush” conditions in Arctic Alaska. **Please inquire if supervision is currently available**
Integrated Care Counselor II:
Has met all minimum qualifications for Integrated Care Counselor I. In addition has obtained at least a Master’s Level professional mental health license in Alaska. Examples of qualifying Master’s level licenses include Licensed Professional Counselor (under AS 08.63) and Marital and Family Therapy (under 08.29). Other qualifying Master’s level licensures are (1) Licensure as a Psychological Associate or (2) Licensure as a Master of Social Work. Candidates with qualifying Master’s level licensures will still be required to obtain their terminal Licensure (LAP, or LCSW) within two years. *Please inquire if supervision is currently available**
