What are the responsibilities and job description for the CCS Service Facilitator position at MANITOWOC COUNTY?
Working in our Clinical Services Division of the Human Services Department, this position facilitates services for adults, children and adolescents who have been diagnosed with a mental illness and/or substance use disorders. This position:
- Performs functional screens and comprehensive bio-psycho social assessments for children’s referrals to determine eligibility and service needs of the child.
- Acts as service facilitator by developing teams of formal and informal supports, collaborating with schools, parent's, foster parents and service providers, developing service plans, make referrals to other agencies and coordinating contracted services.
- Provides ongoing case management and service coordination to families and children enrolled in CCS.
- Develops crisis plans and provides crisis services.
- Facilitates referral, admission, transfer and discharge arrangements with inpatient and residential facilities and providers.
- Generates and maintains clinical records and reports per state standards and agency policy
- Perform timely documentation within 2 business days and maintain required productivity rates.
