What are the responsibilities and job description for the CLTS Waiver & CCS Specialist position at MANITOWOC COUNTY?
Manitowoc County is seeking a professional to join their CLTS/CCS team. This position authorizes and coordinates services to assist families to care for their children in their home that are dually enrolled in Children's Long Term Support (CLTS) and Comprehensive Community Services (CCS) and:
- Coordinates and collaborates closely with the Comprehensive Community Services team.
- Processes referral forms, interviews collateral sources, and administers functional screens to determine if the individual is eligible for services and the level of care and services needed.
- Provides families with options of outcome based goals and creates individual service plan.
- Provides case management through contacts with families and service providers. Participates in collaborative meetings and attends appointments.
- Teams with Comprehensive Community Services (CCS) service facilitators as part of CCS service plan.
