Care Coordinator
Full-Time / Non-Exempt / Havre
Job Duties :
- Administers vital signs, review medical histories with clients, conduct UA and Saliva testing, triage phone calls, and client scheduling as needed.
- Ensure client`s care is coordinated between client and all healthcare providers, provides continuous collaboration with local agencies to establishment continuity of care for the clients.
- Ensures clients` access to needed services during transitions from different levels of care.
- Provides education, reinforcement, and frequent communication with clients about their physical and behavioral health status.
- Provides encouragement and support to clients to assist them in solving difficulties they may have managing and improving their health.
- Grant funded position ending September 29th 2027.
Minimum Qualifications :
2,000 Sign-On Bonus!!
Hourly Wage : $16.73 DOE
Benefits :
All Positions at Many Rivers Whole Health must pass Background Checks and Motor Vehicle Record Check as well as a background check with DPHHS. Will be required to take Sexual Abuse Training.
