Job Summary : The CNA is responsible for assisting the licensed nurse in the delivery of resident care. Under the direction of the nurse, the home health aide provides medically oriented task(s) to maintain health or to facilitate treatment of an illness or injury provided in a person's place of residence.
Job Qualifications :
- Must meet and maintain State requirements of a certified nursing assistant.
- Requires level of knowledge normally required in the course of obtaining a high school diploma.
- Experience working the long-term care setting favored.
- Completion of on-site orientation program will be given to obtain desired entry level competency.
- Work effectively with general supervision, ability to organize and manage own work load in a busy environment. Some independent actions are required.
- Knowledge of resident care plans, and state home care regulations.
- Ability to communicate well with residents and families promoting customer service and respect.
- Must maintain a calm and warm working environment with co-workers.

Hours of Work : As Scheduled. Works weekends and holidays as scheduled.
DUTIES : RESIDENTS
Reports Vulnerable Adult issues to their director supervisor as necessary.Visits with residents routinely regarding cares, activities, menus, likes and dislikes etc.Offers resolution or a visit from the appropriate individual who may offer resolution to any dislikes or concerns and communicates those with direct supervisor.Ensures any grievance is documented and reported to either the supervisor and / or facility DON / RN.Incorporates customer orientated approach into daily routine.Maintains resident's confidentiality and respect according to HIPPA and Resident Rights.OPERATIONS
Follows the resident's specific plan of care to ensure highest level of health, safety and quality.Follows and adheres to Residents RightsResponsible for meeting the needs and comfort of residents to include bathing, dressing, grooming, eating, transferring, walking, toileting, bed mobility, behavior and treatment in accordance with policies and procedures.Assist with ambulation, transfers and exercises.Assist with body positioning or transfers of clients who are not ambulatory.Assist with bowel and bladder control, bladder training, and incontinent products.Report changes in the client's condition and needs to the nurse in charge.Complete the appropriate records to document care given and pertinent observations.Observe for Environmental cleanliness, pick up and discard any loose garbage, clean and tidy up residents rooms, wheel chairs, walkers or other areas as necessary throughout the work period. Including disinfecting when necessary / scheduled.Follow proper Infection control policy and procedureProvide proper grooming nail care, hair care, oral care, bath schedule, appropriately dressedAnswer Call lights timely with courtesy and respect, knocking before entering the room and introducing self to the resident and / or family / guests in the room.Ensure Dignity and privacy is maintained while providing service to the residentsProvide accurate and honest ADL documentation as delegatedEnsure bath schedule is adhered toDocument skin observations (any changes, new occurrences, or resident concerns)Assists with Resident's hands washed before each mealDevelops a good working relationship with the facility LPNs, RNs, and fellow co-workers.Follow safety procedures, policies and manufactures instructions applicable to assigned duties.Responsible for prevention of unsafe work situations.Attends training and in-service opportunities.Assist with residents with ambulation and / or exerciseServes meals & snacks to residents and feeds clients who require assistance or who are at risk of choking.Sets up dining room for meals.Collects dishes and cleans dining room after meals.Washes dirty dishes, dries and returns to proper storage.Places soiled laundry in dirty hoppers, transfers to laundering area for washing.Joins resident during an outing when necessary. (i.e. appointment, group activities)Completes house hold tasks; per task list assigned to each shift.Completes other duties as designatedMedication Administration :
Home Health Aide / Unlicensed personnel may administer medications, whether oral, suppository, eye drops, inhalant, topical, or administered through a Gastrostomy tube if :The medications are regularly scheduledPrior to the administration, the HHA is instructed be a Registered Nurse in the procedure to administer the medications to each client.A Registered Nurse specifies, in writing, and documents in the client's records, the procedure to administer the medications to each client.Prior to the medication administration, the Home Health Aide demonstrates to a Registered Nurse their ability to competently follow the procedure. Competency is documented in the employee personal file.In the case of pro re nata (PRN) medications, the administration of the medication is reported to a Registered Nurse, per policy, and documented appropriately in the RN PRN report form. Insulin may be administered, per policy, when prepared by a licensed nurse.Performance Routine Procedures :
Home Health Aide may perform delegated medical nursing procedures if :Prior to performing the procedures, the HHA is instructed by a Registered Nurse in the proper methods to perform the procedures with respect to each client.A Registered Nurse specifies, in writing, specific instructions for performing the procedures for each client.Prior to performing the procedures, the HHA demonstrates to a Registered Nurse the HHAs ability to competently follow the procedures.The procedures for each client are documented in the clinical record.Job Posted by ApplicantPro
